Camp Cedar Sage Registration

Camp Cedar Sage is a leadership camp for rising 5th-8th grade girls.  It is held June 3-7, 2024 from 8:30 am to 5:00 pm at Camp Copass on Lake Lewisville.  Registration ends on May 1, 2024.  Early sign up is encouraged as we have a waitlist every year.

Follow these steps to secure your daughter's spot for camp this summer:

1. Register your Child

Complete the online registration form below.

​

2. Print, complete and mail forms. 

​

Camp Copass requires hard copies of the two forms below for each camper.  *Hard copies must be received by May 15, 2024 or a late fee of $10 will be assessed.

​3. Submit Payment
 

Camper fee: $450  *Early bird pricing of $425 is available until April 1st.

Payment can be sent via zelle to campcedarsage@gmail.com or check payable to "Camp Cedar Sage."

Camp Cedar Sage Online Registration Form

Physician Release


I, the undersigned parent/legal guardian of the girl named above, hereby delegate authority to the Directors of Camp Cedar Sage to arrange whatever medical treatment they deem necessary for her during her stay at the camp. I also give my permission for any emergency medical care or treatment by a physician, surgeon, hospital, or medical care facility that may be required, including transportation, and accept responsibility for the cost.


 

Photo Release


Also, I hereby authorize and consent the use and reproduction by Camp Cedar Sage staff or an authorized agent or assignee of any and all photographs taken of my daughter for the purpose of promoting Camp Cedar Sage, without any compensation to me. All film, together with any prints, shall constitute property of Camp Cedar Sage, solely and completely.


 

Recognition and Assumption of Risk Agreement/Hold Harmless


I, the undersigned parent/legal guardian of the girl named above, authorize said child’s full participation in Camp Cedar Sage. It is my understanding that participation in the activities that make up Camp Cedar Sage is not without some inherent risk of injury. As such, in consideration of my child’s participation in Camp Cedar Sage, I hereby release, waive, discharge, and covenant not to sue Camp Cedar Sage, its officers, servants, agents, or employees from any and all liability, claims, demands, action, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child, whether caused by the negligence of the releases, or otherwise while participating in such activity, or while in, or upon the premises where the activity is being conducted.  I understand that Camp Cedar Sage cannot be responsible for lost or broken items.  I understand my daughter will comply with all policies and procedures. I also understand, and will comply with all cancellation policies and procedures.

Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19

Camp Cedar Sage has put in place preventative measures to reduce the spread of COVID-19; however, Camp Cedar Sage cannot guarantee that you will not become infected with COVID-19. Further, attending activities at Camp Cedar Sage could increase your risk and your family’s risk of contracting COVID-19.

 

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my family and I may be exposed to or infected by COVID-19 by attending activities at Camp Cedar Sage and that such exposure or infection may result in personal injury, illness, permanent disability, and even death. I understand that the risk of becoming exposed to or infected by COVID-19 at Camp Cedar Sage may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Camp Cedar Sage employees, volunteers, and program participants and their families.  I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself or my family (including, but not limited to, physical illness, disability, and even death),medical expenses, lost wages, expenses or damages of any other kind, involving my person or my property (hereinafter “Claims”) that I or my family may experience or incur arising out of my participation in Camp Cedar Sage activities. On my behalf, and on behalf of my family, I hereby release, covenant not to sue, discharge, and hold harmless Camp Cedar Sage, its officers and directors, employees, volunteers, agents, attendees, contractors, guests, and invitees, of and from any Claims arising from my participation in Camp Cedar Sage activities. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Camp Cedar Sage, its officers and directors, employees, volunteers, agents, attendees, contractors, guests and invitees, whether a COVID-19 infection occurs before, during, or after participation in any Camp Cedar Sage.
